As others have stated, it was just a visual joke or sight gag. They could fit it in almost every episode and have fun with it. On the final show ending special (which should be avoided at all costs since it was seriously not funny) Wilson does appear with his face fully visible, and for a moment it’s hard to recognize him.

I Think It’s Just One Of Those Continuity Gags That Makes It So The Show Keeps Some Repetitiveness. There Is One Episode Where It shows The Bottom Half Of His Face But Not The Top Half.

simply to keep you trying to see the rest of his face… adds mystery to the character

I think he was injured during the Vietnam War but I have seen his entire face before……
